How do I get started

Call our office at (815) 436-1530 or Request an Appointment to get started. During your first visit, Dr. Justina will determine what plan you qualify for and our team will help you join the program while you are in the office.

Can I use my membership for my first visit?

Yes! As soon as you are added on the platform, your membership becomes active immediately.

What is the $50 dollar life-time activation fee?

It is a one-time activation fee.

What forms of payments are accepted

We accept major credit and debit cards.

How can I use my member exclusive discount for restorative or elective treatment?

Simply schedule an appointment. After we verify that your membership is “current”, you will be automatically given a 25% discount on certain services.

What services my exclusive membership does not cover?

Anything related to cosmetic procedures, such as teeth whitening, botox, and fillers. Call our office and ask about a specific service and whether it qualifies.

What if I miss an appointment?

It will be your responsibility to reschedule. Our practice policy allows a 24-hours cancelations. If you miss an appointment (no show or last minute cancelation), $75 fee will be added to your account.

Can my family members join as well?

Absolutely! You and your family can join the savings plan, but each individual, including a child, must register separately as a new member.

Can I use this membership anywhere else?

No. It is exclusive membership at Pinnacle Dental Group only.

How do I update my billing and other information?

All changes can be done by logging in to your account.

Can I cancel anytime? How?

Login to your member portal to cancel your account. No refunds are issue for the past payments at the time of cancelation.

Can I pay annually for my membership?

No. Memberships are month-to-month at this time.



Our dental savings membership plan is not an insurance, but rather a financial arrangement between our patients and our practice to help our loyal patients without insurance. Memberships are provided exclusively to uninsured patients of our practice and shall not be considered pre-payment for future services or payment for access to discounted services. No refunds will be provided for dues paid under any circumstances, including failure to schedule and maintain appointments. Monthly dues and fees for dental services may change at any time.
